This session will explore topics on patient success and retention, and best demonstrated practices for Patient Care Teams including the roles of Home Nurses, Social Workers, Dietitians, and Patient Care Technicians.

Carson Daniel is a Certified Clinical Hemodialysis Technician with 13 years of experience working in UVA Lynchburg Dialysis outpatient unit. Carson recently transitioned into a new role as a PCT in UVA’s Home Training department, assisting in both the home training unit and UVA’s newly added Transitional Start Unit.

Registered Nurse Administrative Coordinator, University of Virginia Lynchburg Home Program

Barbara Jefferson is a certified Dialysis Nurse who specializes in Nephrology at the University of Virginia Lynchburg Home Dialysis Program. As Registered Nurse Administrative Coordinator, she helps create recruit and train patients for home dialysis therapies. Barbara shares her expertise in managing and growing home hemodialysis and peritoneal dialysis program for UVA Lynchburg Home Program. She is also a member of American Nephrology Nurse Association, and serves on the NxStage Nurse Advisory Board.

Milly Greenway, BSN, RN †‡
Nurse Clinical Manager, Fresenius Kidney Care Omaha

Milly Greenway is a clinic manager who specializes in home hemodialysis, peritoneal dialysis, pediatric dialysis as well as transitional care. Milly excels in developing, implementing and managing all aspects of a fast paced home dialysis clinic. She has also successfully planned, developed and launched a transitional care unit within her home clinic to better serve patients and partners in her community. She has shared her expertise through multiple speaking engagements at ADC and Nebraska Kidney Association Joint Council meetings.

Cara Snyder, PCMSW, PLMHP
Social Worker, Fresenius Kidney Care Omaha

Cara Snyder is the Master Facility Social Worker II at Fresenius Nebraska Home Dialysis, serving over 80 patients in both the Home Adult and Pediatric Clinic and the Transitional Care Dialysis Unit. Cara has over 20 years of experience in varying social service settings in large urban and rural areas with a focus on home care and addressing the needs of the homeless population.

Dr. Michael Kraus is Associate CMO, Senior Medical Advisor at NxStage Medical, Inc. Prior to working for NxStage, Dr. Kraus held several positions and titles at IU Health and Indiana University School of Medicine, including: Service Line Chief for IU Health Physicians, Kidney Diseases; and CMO of IU Health Adult Dialysis Services. Dr. Kraus is a leader in the field of short daily home hemodialysis and a well-published, internationally-recognized invited speaker. He is the recipient of a Lifetime Achievement Award from the National Kidney Foundation of Indiana, as well as the Sagamore of the Wabash, the highest award presented to a citizen of Indiana.

Michelle Carver is a certified nephrology nurse who specializes in home hemodialysis and peritoneal dialysis programs across the country. As the Senior Director of Clinical Education for National Accounts at NxStage Medical, she helps create clinical education programs and platforms to support patients on home hemodialysis. Michelle shares her expertise in developing, implementing, and managing home hemodialysis and peritoneal dialysis programs through invited speaking engagements. She is also a member of the American Nephrology Nurses Association (ANNA), and the National Kidney Foundation.
